What it actually is
Starch chemically or physically altered to thicken, stabilise or resist heat. It is classified as a thickener, which describes its function in the product rather than what it is made from — a distinction that causes a lot of confusion, because two additives with the same job can come from completely different sources.

Is it gluten free?
A gluten grain is a realistic source and the finished ingredient can carry protein. Look for a certified product or ask the manufacturer.
In the EU, if the starch is wheat-derived the label must say so — "modified wheat starch" — because wheat is a declarable allergen. Unqualified "modified starch" on a European label therefore means it is not wheat.
How we categorised this
Three categories, not two, because the two-category version is what makes most answers on this subject useless. "Not a gluten risk" means the substance cannot contain gluten — a mineral, or a defined molecule with no cereal anywhere in its production. "Almost always fine" means the usual feedstock is gluten free and, where a gluten grain is used, processing removes protein below the 20 mg/kg legal threshold. "Worth checking" means a gluten grain is a realistic source and residual protein is plausible.
European labelling law does much of the work here. Wheat is a declarable allergen, so an unqualified "modified starch" on an EU label is not wheat — if it were, the label would have to say so. The specific exemptions, for wheat-derived glucose syrup and maltodextrin, exist because the processing evidence supports them [1].
